A mental condition has been officially diagnosed, but the symptoms are not severe enough to interfere with work and social functioning or to require ongoing treatment.
Occupational and social impairment due to mild or transient symptoms that reduce work efficiency and the ability to perform professional tasks only during periods of significant stress, or symptoms that are controlled by continuous medication.
Professional and social impairments with occasional reductions in work efficiency and occasional periods of inability to perform professional tasks (although generally functioning satisfactorily, with routine behavior, self-care and conversation normal), due to symptoms such as: depressed mood, anxiety, suspiciousness, panic attacks (weekly or less often), chronic sleep impairment, mild memory loss (such as forgetting names, directions, recent events).
Professional and social impairment with reduced reliability and productivity due to symptoms such as: flattened affect; indirect, circumlocutory or stereotyped speech; panic attacks more than once a week; difficulty understanding complex commands; impairment of short-term and long-term memory (eg, retaining only highly learned material, forgetting to complete tasks); impaired judgment; impaired abstract thinking; motivation and mood disorders; difficulties in establishing and maintaining effective work and social relationships.

Occupational and social impairment, with deficits in most areas, such as work, school, family relationships, judgment, thinking or mood, due to symptoms such as: suicidal ideation; obsessive rituals that interfere with routine activities; speech is occasionally illogical, unclear, or irrelevant; almost continuous panic or depression that affects the ability to function independently, appropriately and effectively; impaired impulse control (such as unprovoked irritability with periods of violence); spatial disorientation; neglecting personal appearance and hygiene; difficulty adjusting to stressful circumstances (including work or work-like environments); inability to establish and maintain effective relationships.
Total professional and social impairment, due to symptoms such as: gross impairment in thought processes or communication; persistent delusions or hallucinations; grossly inappropriate behavior; constant risk of harming oneself or others; occasional inability to perform daily activities (including maintaining minimal personal hygiene); disorientation to time or place; loss of memory for names of close relatives, own occupation or own name.
A common misconception in the veterans community is that you must meet ALL of the subjective symptoms associated with a particular PTSD rating in order to receive that rating.
A Veterans Review Service representative, also known as an RVSR, will consider all the evidence on record and will usually assign a VA rating for PTSD that includes “overcoming symptoms”.

For example, if a veteran has 4 symptoms out of 70 PTSD criteria scores and 6 symptoms out of 100 PTSD VA criteria scores, the RVSR will assign a higher score, unless record evidence contradicts this subjective assessment.

According to 38 CFR PTSD §4.126, evaluation of disability due to mental disorders, the RVSR is required to consider these two rules:
1. In assessing PTSD, the rating agency will consider the frequency, severity and duration of psychiatric symptoms, the length of remissions, and the combatant’s ability to adjust during periods of remission.

The rating agency assigns a rating based on all evidence of occupational and social impairment, not just the examiner’s assessment of the degree of disability found in the exam.
2. When assessing the degree of disability for PTSD, the rating agency will take into account the degree of social impairment, but will not assign an assessment solely on the basis of social impairment.
